Offspeed Volleyball Waiver

Assumption of risks: I am aware that participation in sports, without limitation, offered by or associated with Offspeed Volleyball and its affiliate programs, exposes my child to many inherent risks. By engaging my child in these activities, I freely accept and fully assume all inherent risks, dangers, and hazards and the possibility of personal injury, death, property damage or loss resulting there from.
Release of liability waiver of claim and indemnity agreement: In consideration of Offspeed Volleyball and its affiliate programs providing my child with the opportunity to participate in their activities, permitting him/her the use of its equipment and the use of facilities, I hereby agree to as follows: 
 

To waive any and all claims that I have or may in the future have against Offspeed Volleyball and its affiliate programs, as well as their directors, employees, agents, representatives, and successors.

To release Offspeed Volleyball and its affiliate programs, as well as all their directors, employees, agents, representatives, and successors from any and all liability for any loss, damage, injury or expense that I or my child may suffer as a result of his/her participation in activities and sports offered by Offspeed Volleyball and its affiliate programs; due to any cause whatsoever Including negligence, breach of contract, breach of statutory duty of care and/or breach of the occupier's liability act, On the part of Offspeed Volleyball and its affiliate programs as well as its directors,  employees, agents, representatives, and successors.

To hold harmless and indemnify Offspeed Volleyball and its affiliate programs, and all their directors, employees, agents, representatives, and successors from any and all liability for any property damage or personal injury to any third party, resulting from my child’s activities and his/her participation in the activities offered by or associated with Offspeed Volleyball.

Personal information: I understand that Offspeed Volleyball and its affiliate programs are collecting certain personal information about my child, me and other members of my family. I also understand that this information will be used only for the purpose of administrating Offspeed Volleyball and affiliated programs. I hereby consent to such collection, use and disclosure of this personal information.

I also authorize Offspeed Volleyball and its affiliated programs to take photographs and or video of my son/daughter or myself during their participation (or mine) in the sport to display them and otherwise use these photos or videos without charge solely for the purpose of promotional material in connection with Offspeed Volleyball. These may be for print ad, display on their web site, or other promotional use as the organization may determine.

Finally, I understand that No filming of athletes is permitted at Offspeed Volleyball training sessions unless a recognized program videographer. Any parent found videotaping children at a volleyball training session is subject to having their child removed from the program. No additional warnings will be provided.

By completing our online registration, it is understood that I have read and accept the above listed conditions.
